Webcasting of Presentations
Live web streaming will be used to broadcast the presentations to the general public. The stream will consist of slides and audio only. We are requesting that you confirm agreement with this form to include the live streaming of your presentation.

I, the undersigned, hereby grant to the ISMRM/SMRT permission to Webcast my presentation by live Web streaming to the general public. This includes all course content within my presentation as slides and audio only.

I, the undersigned, understand that the Content of CME activities will be restricted to pure science, industry issues and operation of devices, and should not include any advertising, corporate logos, trade names or a product group message of an ACCME-defined Commercial Interest either within the presentation or my disclosure of financial interests as part of my presentation. Should there be a perceived conflict of interest regarding my presentation slides, I agree to work with ISMRM to resolve the conflict as soon as possible prior to presentation/posting of educational materials.
